| | Phone or in-person appointment? |
Phone appointments are available, but they are not right for every health concern. Some health problems cannot be assessed over the phone. To give you the right care, your provider may need to see you in person. Please request an in-person appointment if you have: - A possible infection
- A skin change, rash or wound
- Any new pain (for example, in your stomach, joints, back, head or throat)
- A cough
- Dizziness or balance problems
- Heart symptoms, such as a fast heartbeat
- Any other symptom that may need more testing or a new medication
To change your appointment to in person, please call our front desk at 416-658-6812. In an emergency, such as severe chest pain or serious trouble breathing, call 911 or go to the nearest emergency department. | | New self check-in tablet |
Patients with an Ontario health card can now check in for appointments using the tablet beside the front check-in window. The process is quick and simple. If you prefer, our medical secretaries are always happy to check you in. | | Welcome to our new registered dietitian |
We are pleased to welcome Vanessa, Registered Dietitian, to our team. Vanessa can help you build a healthy eating plan that fits your needs, budget and culture. To book an appointment with Vanessa, please ask your provider for a referral. | | Case management supports |
Our case managers can help you find the support and resources that you need. They can assist with: - Applications for social assistance, disability benefits and other government programs
- Finding food banks, community resources and housing supports
- Completing applications, forms and other documents
Drop-in hours: Mondays and Wednesdays, 3 p.m. to 5 p.m. (no appointment necessary – just drop in and ask to see a case manager at 1900 Davenport Rd.) Appointments: Please call 416-656-8025 to book.
