| | Review Your 2024 Clinical Integration Index Score Report | |
Your 2024 Clinical Integration (CI) Index Score Report is now available and summarizes your performance in the 2024 CI Program.
Please review the communication we sent on August 6 for details on where to find your report in your practice’s OneDrive account, as well as instructions for submitting supporting documentation if you choose to participate in the supplemental file submission process. All submitted materials for both Adult PCPs and pediatricians are due by this Friday, August 22.

QPM Tip:
Kidney Health Evaluation for Patients with Diabetes (KED)
| | In addition to a blood test, providers must order a urine albumin-creatinine ratio (uACR) test yearly in order to close KED care gaps. Separate urine creatinine and urine albumin tests alone will NOT satisfy the measure without a calculation of the ratio of albumin to creatinine. Make sure to provide urine collection cups at check-in for all patients with diabetes if urine tests are collected in-house. Complete your CI Program requirements to remain an active member of the Clinically Integrated Network (CIN).

- Preauthorization, pharmacy, and medical policy updates
- OrthoNet portal will remain active until further notice
- Claims information, including contacts, electronic submissions, and timely filing information
| | Use your Patient Opportunity Reports to close care gaps | |
On July 2, adult PCPs received updated Patient Opportunity Reports in their practice's main OneDrive account. Use these patient-level reports to identify patients that need visits to re-engage in care, close quality care gaps, and/or manage and document chronic conditions.

We are now over halfway through the year – now is a great time to ensure you're on track to close open care gaps and bring in patients not yet seen this year.
Please reach out to your provider engagement team member if your practice needs any additional support on improving workflows to address open care gaps or how to use the reports.

To enhance security, the option to receive a text message (SMS) code for login to the MSHP Provider Portal has been removed. Instead, you will need to answer the security question you created for your account.
If you remember the answer to your security question
You’re all set! Simply use that to login moving forward.
If you do not remember the answer to your security question
You will need to email us at mshp@mountsinai.org so we can submit a ticket on your behalf to reset your security question.
Please review your login method as soon as possible to ensure you're able to log in to the Provider Portal.
