Important Information on How To Log-In For Covid-19 PCR Test Instructions
When you receive the shipment from Guardian Logistics containing PPE and COVID-19 test kits, you will need the following information!

Follow the steps below to access the instructional materials for the COVID-19 testing and registration:

  • Click HERE to open the spreadsheet for your Premier Labs Username/Password.
  • Access the Premier Labs portal HERE.
  • Enter your username and password.
  • Select the appropriate instruction manual below (either CLIA or non-CLIA) for your facility. Page 7 includes PCR registration instructions.
  • CLIA Waiver facilities, click HERE.
  • Non-CLIA Waiver facilities, click HERE.

Reminder to all LTC facilities: Facilities using a point-of-care antigen test device for detection of SARS-CoV-2 are required by State Health Director Order to report all results, positive and negative, of diagnostic testing for SARS-CoV-2, the virus that causes COVID-19. This includes antigen test results. Facilities that are reporting detailed results through Electronic Laboratory Reporting (ELR) or COVID-19 Laboratory Data Automation (CLDA) should include all Antigen testing in your reporting results.
