Ready to Mobilize for Action? Inform Our 2027 - 2031 CHIP Process!

The Live Well Lake County Network is excited to announce the launch of our next Community Health Improvement process using the MAPP 2.0 Framework! MAPP stands for Mobilizing Action through Planning and Partnerships and is a community-driven strategic planning process to achieve health equity. MAPP was developed by the National Association of County and City Health Officials (NACCHO) in 2001 and is one of the most widely used and reputable community health improvement (CHI) frameworks in public health. MAPP provides a structure for communities to assess their most pressing population health issues and align resources across sectors for strategic action. It emphasizes the vital role of broad stakeholders and community engagement, the need for policy, systems, and environmental change, and alignment of community resources toward shared goals.

The process includes three phases as shown below. Phase I will be from March-June 2025 and involves assessing the current CHIP. Phase II will be from April 2025-January 2026 and involves completing three community health assessments, identifying themes, developing issue profiles that outline data and information about each issue, and sharing findings. Phase III will be from January 2026-July 2026 and involves prioritizing health issues, developing shared goals, selecting CHIP strategies, developing action planning cycles, and monitoring progress. We value and need community input and involvement. Stay tuned for a variety of opportunities to participate and make your voice heard!

Network Resources

The PLE Work Group is working on a number of one-pagers to support Network member engagement. This month's resource provides information about how to maximize your participant experience using the Zoom platform. Watch for English and Spanish versions of one-pagers like this one on the Live Well Lake County website!

Network Updates

Announcing Live Well's Storytelling Working Group Co-Leaders - Shonta & Idriss Joseph

The Storytelling Working Group was established to make Network activities and the CHIP more accessible to our community through story.

We are delighted to announce Shonta and Idriss Joseph will serve as our Storytelling Working Group Co-Leaders! The Josephs are no strangers to storytelling and each of them brings their unique experience and expertise to the group. Shonta has over a decade of acting experience, including improv and TV, and is passionate about encouraging others to find and share their authentic voice through storytelling.

Idriss has dedicated much of his career to giving voice to underrepresented communities and driving conversations around social justice. He also hosts Mindset Reset, a weekly virtual meeting focused on creating a safe space for Black men.

Additional members include Venoncia M. Baté-Ambrus, Vavi Solomon and Manny Hernandez. If you are interested in joining this group, please contact Shonta Joseph at shewise2023@gmail.com.
