Our Next-Generation HIE Coming Soon
An Integrated, Efficient, User-Friendly Platform
| | |
HealtheConnections is entering the final phase of preparation for the launch of our upgraded health information exchange (HIE) platform, which will replace the myConnections clinical portal. This enhanced system will deliver stronger performance and a more seamless user experience, designed to support more effective, connected care across our region.
We’re excited to introduce these improvements and appreciate your continued partnership. If you’re a current user, the changes you will experience are primarily in the design of the user interface and exciting upgrades to our alerting platform – all other workflows that you have already established will not need to change. Please watch your email for regular updates in the coming weeks.

HealtheConnections Announces SHIN-NY Statewide Encounter Alerts
Improving Care in NY with Real-Time Insights
| | |
HealtheConnections is excited to announce the launch of a next-generation encounter alerting solution, part of a major statewide initiative to deliver faster, secure, and consistent patient-centered notifications to all New York participants.
Through a new contract awarded by the Statewide Health Information Network for New York (SHIN-NY), HealtheConnections will provide SHIN-NY Statewide Encounter Alerts, giving healthcare providers real-time notifications of hospital and emergency visits—no matter where in New York State the encounter occurs.
As part of this effort, HealtheConnections will now also be able to offer this enhanced alerting solution to SHIN-NY participants outside of our region, expanding access to more providers across the state. This supports New York’s vision for a more connected, patient-focused healthcare system. Enhanced statewide alerting is a key step in strengthening interoperability and care delivery across the state.

Powering Connections to Meet Social Needs
HRSN Data is Flowing Across the Region
| | |
We’re thrilled to announce a key milestone in advancing the 1115 Medicaid Waiver initiative: thousands of Health-Related Social Needs (HRSN) datasets are now actively flowing into the statewide SHIN-NY Data Lake from across the region. This achievement reflects the power of collaboration among healthcare providers, Social Care Networks (SCNs), and the Unite Us platform, marking a major step forward in the 1115 Waiver initiative's progress.
As part of the SHIN-NY, HealtheConnections has partnered with other Qualified Entities to play a vital role in making this possible. We have helped build the trusted data infrastructure that supports secure, connected, and coordinated care across our region and throughout the state.
Together, we have delivered:
| |
- 7,000 HRSN Screenings
- 2,915 Comprehensive Assessments
| |
- 5,793 Open Referrals
- 7,487 Confirmed Services
| | |
...and we’re just getting started.
What’s Next: EHR Integration in Motion
We’re building on this momentum with early integrations from electronic health records (EHRs), beginning with Epic and Medent. These connections will enable providers to submit HRSN data directly through their EHR systems, streamlining workflows and aligning with statewide data-sharing goals. Development is underway, and we welcome organizations already collecting HRSN data in their EHR or are interested in participating to connect with us.
Multiple Pathways to Share HRSN Data
Providers can choose between several options for exchanging HRSN data that best fits their workflows:
Let’s work together to expand access, improve care coordination, and drive meaningful impact through connected care.
