In Pursuit of Understanding the Crash
Myalgic encephalomyelitis is characterized by post-exertional malaise (PEM). PEM is variously defined, but it involves an abnormal multi-system and multi-symptom response to often-trivial levels of exertion. It frequently results in severe disability. Everyone’s specific pattern of signs and symptoms is different, but they all have in common a worsening after exertion. Solving PEM would solve ME/CFS. Read more here.
Cardiopulmonary Exercise Test Explained
In a previous post, I described how cardiopulmonary exercise testing (CPET) has changed the game in myalgic encephalomyelitis/chronic fatigue syndrome (ME/CFS) research and clinical practice. CPET has provided a wealth of objective physiological evidence supporting what patients have always told us. When done two days in a row, CPET can be used to assess the failure to recover after exertion, which appears central to ME/CFS pathophysiology. Read more here.
