Provider Directory Verification

Please verify that HWMG’s provider directory correctly reflects your practice information (name, gender, specialty, office locations, phone numbers, accepting new patients, etc.).

  1. In the HWMG provider directory, type your first and last name or practice name in the search fields.
  2. Click the Search button and review your practice information.
  3. Submit a verification form online (linked below) to confirm our provider directory correctly reflects your information. If any information is inaccurate, please upload a Provider Add, Delete or Change Form.

We also ask that you also inform HWMG of any changes to your billing and mailing information including addresses (especially suite numbers), phone numbers, email addresses, and removal of providers no longer practicing due to retirement or other reasons. You may use the verification form linked below.

Incorrect information on file may affect members’ access to care and delay your receipt of information from us.

Please contact Provider Relations if you have questions. Thank you for your cooperation!

Join Us at NAMIWalks Hawaii

Saturday, October 10, 2026 | Kaka`ako Waterfront Park | 9:00 am

HMAA proudly supports NAMIWalks Hawaii! NAMIWalks is America's fundraising event that walks the walk - powering NAMI for the important work it does in our local community. It's a celebration of individuals, families, and communities coming together for one powerful purpose: Mental Health for All.

We invite you to visit our Wellness table on Saturday, October 10 at Kaka’ako Waterfront Park. NAMIWalks will also be held on Hawaii Island, Kauai, Maui, and Lanai that day.

The National Alliance on Mental Illness is the nation’s largest grassroots organization dedicated to building better lives for the millions of Americans affected by mental illness. We invite you to put on your walking shoes and help us raise awareness and funds for mental health support and vital programs.
