Hello Families,

We are so thankful for another wonderful year caring for your children. We hope this summer is filled with family time and, most importantly, well-deserved rest! Please be on the lookout for an email from Final Forms – sometime around the end of June or early July – regarding mandatory health forms for the upcoming 2026-27 school year. If your children have received any new immunizations, please upload updated copies of their vaccination records in Final Forms. This short instructional video shows you how to upload the document.

These medical forms must be completed by August 1. Please note that these are not athletic physicals. Let us know if you have any questions or concerns.

For those students who keep emergency medications and/or daily medications in the Nurses’ Office, we have placed those in your student’s backpack.

Lastly, for parents whose children use an Epipen or AviQ, there is now a nasal epinephrine spray available with a doctor’s prescription. Please click here for more information.
