Immunization Requirement

Incoming 12th Graders

Immunization Requirement: Meningococcal vaccine is required for all 12th graders enrolled in Delaware City Schools for the 2025-2026 school year.

Ohio law mandates that all students have two doses of the meningococcal vaccine prior to entering the 12th grade. If the first dose was given on or after the 16th birthday, a second dose is not required.

If your child has not yet received these vaccinations please schedule to have it administered now. You can find information on the Delaware Public Health District website at https://www.delawarehealth.org/ about the schedule of vaccination clinics, or you can call them at (740)368-1700 to make an appointment. Meningococcal vaccines are also available through your child's doctor, local pharmacies, and clinics.

Students who have already had the second dose of the meningococcal vaccine, should turn in documentation, or an exemption letter from a parent or physician now. Please call the clinic if you have any questions about the vaccine or your child’s current immunization record at school. The clinic direct line is 740-833-1019. The school fax number is 740-833-1099.

Forms should take less than 10 minutes per student.
