Kentucky Health Survey Registry
The annual utilization surveys are available for submission of your 2024 utilization data. The survey must be completed by March 15, 2025. Please go to https://prd.webapps.chfs.ky.gov/oigpublic/ to complete the survey. Logging in to the survey requires an identification number (the facility’s license number) and a password. If you are unsure of the password or have questions, please email Michele Bushong at Michele.Bushong@ky.gov.

Frequently Asked Questions Document and Methodology Report for the Screen Positive for Health-Related Social Needs Indicator Confidential Feedback Report (available in HH, IRF, and LTCH)
CMS has released a Frequently Asked Questions (FAQ) Document and Methodology Report to answer key questions and provide methodological information about the Screen Positive for Health-Related Social Needs (HRSN) Indicator Confidential Feedback Report.* To access these and other materials relating to the Screen Positive for HRSN Indicator report, please go to the following Post-Acute Care Quality Reporting Program Training & Education pages:
*CMS released the initial Screen Positive for HRSN Indicator report to Home Health, Inpatient Rehabilitation Facility, and Long-Term Care Hospital providers in October 2024. CMS plans to release this report to Skilled Nursing Facility providers beginning in October 2025, when a full Fiscal Year of data (October 1, 2024 – September 30, 2025) will become available for the first time.

Public Reporting January 2025 Refresh of HH QRP Data
The January 2025 quarterly refresh for the Home Health Quality Reporting Program is now available on the compare tool on Medicare.gov and the Provider Data Catalog (PDC). For this refresh, Home Health (HH) Outcome and Assessment Information Set (OASIS) will be based on the standard number of quarters.
In addition, three OASIS measures have been added for the January 2025 refresh:
- Transfer of Health Information to the Provider
- Transfer of Health Information to the Patient
- Discharge Function Score
The January 2025 refresh also includes the removal of one OASIS measure:
- Application of Percent of Patients with an Admission and Discharge Functional Assessment and a Care Plan that Addresses Function Process
CMS has also posted results from the calendar year (CY) 2023 performance year for the expanded Home Health Value-Based Purchasing (HHVBP) Model. The data can be viewed on the PDC.

Updates to Providing OASIS Privacy Notice
As required by the HH Conditions of Participation (COP), an OASIS privacy notice must be provided to all patients for whom the OASIS data is collected. Due to the transition to OASIS data collection for all patients, regardless of payer, effective January 1, 2025 Home Health Agencies should only provide patients with the CMS Privacy Act Statement and Attachment A - Statement of Patient Privacy Rights. These forms are available on the Home Health Agency Center webpage in the OASIS section. Both documents are available in English and Spanish.
Beginning January 1, 2025, and until further notice, Attachment C – Notice About Privacy for Patients Who Don’t Have Medicare or Medicaid should not be provided to patients.

The OASIS Answers, Inc. (OAI) home health training webinars are designed to provide the most innovative training to your clinicians, clinical leaders, quality improvement teams and auditors. Interested in Serving on an IDR Panel?
Kentucky requires one member of the IDR panel to be "a person currently engaged in the provision of Home Health Agency services who has no affiliation with the provider disputing a deficiency." Typically, a Home Health Agency Director is on each IDR panel. There is no payment or travel reimbursement for a Director serving on a panel. Directors will be rotated. If a Home Health Agency Director is interested in serving on a panel, a confidentiality statement must be signed prior to the participation in the first IDR panel.

Home Health Agency Questions and Answers
Home Health Agency (HHA) Providers can access the Home Health and OASIS Q&As and other reference manuals on the HHA Providers Reference site.
These questions and their responses have been developed over many years of experience training OASIS coordinators and are currently used as part of the Home Health Agency Surveyor Training program. They can and should be used by home health agency providers in training new staff on the basics of OASIS and also as a refresher for existing staff.
