IMA Physician Leaders Advocate for Healthcare Reform on Capitol Hill

On February 23 - 25th, IMA leaders participated in the American Medical Association (AMA) National Advocacy Conference in Washington, DC.

IMA President Dr. Megan Kasper, Idaho AMA Delegate Dr. Patrice Burgess, Alternate Delegate Dr. Zachary Warnock, and IMA CEO Susie Keller met with Idaho's Congressional Delegation to champion AMA reform efforts in three crucial areas: Medicare physician reimbursement, reducing prior authorization burdens and protecting access to care in Medicaid. AMA research on these policy priorities indicates that:

  • Since 2001 Medicare physician payments, when adjusted for inflation, have dropped 33%. Other Medicare providers (e.g., hospitals, Medicare Advantage) receive statutory based inflationary updates, but physicians are excluded.

  • 40% of physician survey respondents hired staff to work exclusively on prior authorization requirements, and more than 1 in 4 physicians (29%) report that prior authorization requirements have lead to a serious adverse event for a patient in their care, including hospitalization, permanent bodily damage, congenital anomalies/birth defect, or even death.

  • Changes to Medicaid financing are entering the implementation phase, warranting support and technical assistance to states to ensure smooth transitions for patients and reduce provider instability.

IMA Supports Prior Authorization Legislation

IMA CEO Susie Keller featured on The Ranch Podcast

Susie Keller is the CEO of the Idaho Medical Association. In this discussion, Susie explains the provisions of several healthcare policy bills currently under consideration by the Idaho Legislature, including HB 611, which seeks to regulate the prior authorization process.

Susie notes that existing prior authorization requirements lack uniformity and transparency, resulting in administrative burdens and delays in care. National data shows that 93% of the time there is a delay for the patient to receive the care that their physician wants them to have, whether that's a service or a prescription. Other states have successfully implemented prior authorization reform legislation that has eased these burdens; now Idaho has a chance to do the same with HB 611.

IMA is in strong support of HB 611, which would require insurance companies to operate more transparently with respect to their prior authorization requirements, mandate response time parameters for prior authorization requests, and require that appeals of prior authorization denials would be handled by a physician in a similar specialty to the physician requesting the care.

Susie addresses other key legislation followed by the IMA Advocacy team, including legislation on parental consent, vaccine requirements, and more.

The Ranch Podcast, hosted by Eagle resident Matt Todd, is conversational and informative, encompassing a broad spectrum of topics in a non-confrontational format that invites viewers to hear from speakers with varying perspectives on issues impacting the daily lives of Idahoans. Medicare Telehealth Extensions Announced through 2027

After years of short-term extensions and looming deadlines, Congress has finally delivered a measure of stability for Medicare telehealth. The Consolidated Appropriations Act, 2026 (H.R. 7148), signed into law early February, retroactively extends the Medicare telehealth waivers that briefly expired on January 30, 2026, and continues those flexibilities through December 31, 2027. These waivers, first implemented in 2020 during the COVID-19 public health emergency, have become deeply embedded in how providers deliver care and how patients access it. the nearly two-year extension is significant because it reduce near-term uncertainty and allows providers to maintain current operations.

The following Medicare telehealth waiver provisions are now extended through December 31, 2027:

  • Waiver of geographic and originating site requirements, allowing beneficiaries to receive telehealth services regardless of location or site of care
  • Expanded list of eligible telehealth providers
  • Continued eligibility of Federally Qualified Health Centers (FQHC) and Rural Health Clinics (RHCs) to furnish telehealth services
  • Delay of the prior in-person visit requirement for mental health services when permanent telehealth policy requirements are not met
  • Continued allowance of audio-only telehealth services
  • Continued use of telehealth to conduct required face-to-face encounters for hospice recertification

In addition, the Acute Hospital Care at Home Initiative was extended through September 30, 2030, further reinforcing federal support for alternative delivery care models outside traditional inpatient settings.

Beyond extending existing waivers, the legislation includes several forward-looking telehealth provisions. CMS is required to establish billing modifiers by 2027 to identify telehealth services delivered through third-party virtual platforms and services billed "incident to" another professional service. Hospitals may continue furnishing and billing for cardiopulmonary rehabilitation services delivered to outpatients in their homes via live video through calendar year 2027. The Department of Health and Human Services must also issue guidance within one year on best practices for delivering telehealth services to patients with limited English proficiency, and CMS must educate clinicians by January 1, 2028 on screening for medication-induced movement disorders, including telehealth-specific considerations and billing guidance.

With enactment of the legislation, CMS has updated its Calendar Year 2026 Telehealth FAQs to reflect the restored flexibilities and new expiration dates. However, the Medicare Learning Network (MLN) Telehealth and Remote Patient Monitoring guidance—last revised in December 2025—has not yet been fully updated to reflect these changes. As a result, some provisions in the MLN, such as references to an in-person mental health visit within six months, are now outdated and require revision.
