We are reaching out to provide updates on the CoC Bonus (CA1341) grant jointly managed by HSH and DMH.
Starting November 1st, 2026, administration of this program will be streamlined through the HSH Referral Access and Data (RAD) team and DMH Federal Housing Subsidies Unit (FHSU).
Changes to expect:

Program management: All programs will be consolidated under the new “CA1341 HSH Bonus- Catch all” or “CA1341 DMH Bonus.” Data entry access will be limited to HSH and DMH staff only. Respective teams will manage program enrollments and exits within HMIS.
- Quarterly reports: ICMS point of contact will receive an email requesting a short update every 3 months.
- Annual assessments: Poins of Contact will be emailed in advance. Please follow instructions as provided.
Processes that will remain the same:
- Re-Certification: HACLA will reach out to ICMS directly for recertification support.
- Documentation: Case noting process will remain the same. Please follow HSH guidelines.
- Client exit request: Exit request process will remain the same. Please follow HSH guidelines.
