Scott Wallace, LICSW - Director of Behavioral Health Operations
Scott was recently named Director of Behavioral Health Operations for TMIN. In this new role, he will lead the TMIN Behavioral Health Program—as part of the broader Tufts Medicine Behavioral Health service line that is being built—and serve as the primary Tufts Medicine relationship owner for Synchronous Health, our teletherapy partner.
This new role reflects the significance of behavioral health within our primary care framework and will be critical in fostering cohesion within the team, functioning as an integral component of the overarching ambulatory care management model. This collaborative effort also extends to Navvis, our partner in Population Health.
Kaitlyn Emond - Care Coordination Specialist Manager
Kaitlyn recently accepted a new role as Care Coordination Specialist (CCS) Manager. CCSs are a critical part of our Elevate Population Health initiative, and offer clinician and practice support services around quality operations and performance. Kaitlyn brings an extensive knowledge and background on quality measures, performance optimization, system navigation, and TMIN structure and history.
In this role, Kaitlyn will lead and oversee day-to-day operations of the CCS team. She will engage our executive-, regional-, LCO-, and practice-level leaders to support quality operations and ensure all components are functioning efficiently to support our clinicians and enhance patient outcomes.

In the News: Tufts Medicine Virtual-First Initiative

Tufts Medicine's new virtual-first initiative will create easier access to care and support the growth of our physician practices.
This initiative will help us meet the increasing demand for immediate and personalized access to care in an environment where many clinicians are at capacity and competitors like Amazon and Optum are introducing similar options for patients. Virtual-first represents an exciting opportunity to attract new patients, retain existing patients and increase capacity at a time of critical need.

Important WellSense Medicaid ACO Updates
|
|
WellSense Care Alliance Prior Authorizations:
Beginning March 1, 2024, WellSense will require that all inpatient and outpatient authorization requests submitted directly to WellSense be performed via digital submission in the WellSense provider portal, also known as HealthTrio. WellSense will no longer accept faxed authorization requests. All clinicians are encouraged to convert their prior authorization submission process as soon as possible.*
- Using the portal, you can submit outpatient and inpatient authorization requests, confirm authorization numbers and check the status of authorizations.

WellSense Care Management Referrals:
If you have a WellSense Medicaid ACO member who needs support for care needs such as social determinants of health, behavioral health or Complex Care Management, please submit your referral using one of the following pathways based on your practice’s EMR:

Epic Practices – submit a referral to care management order via Epic. Click here for guidance on this workflow.

Non-Epic Practices – submit an electronic referral form with the last office visit note via fax or email. Click here to access the Referral to Care Management form.

Practices with Direct Trust/Messaging – submit a Referral to Care Management form from your EMR directly to the TMIN Central Resource Department. Click here to access a guidance document on Direct Messaging.
Moving forward, please stop sending referrals directly to WellSense and use one of the methods listed above. This will assist in centralizing all Care Management referrals and routing to the appropriate discipline(s).

New Social Determinants of Health Training Available

Next month, we will host 6 virtual sessions for all Tufts Medicine clinicians who use Epic on how to collect and document SDOH information in Epic and how to search for community resources using Findhelp – a search platform for finding and applying for free or reduced-cost social services, like food, housing, financial assistance and health care. Use the links below to join the session that works best for your schedule.

Many patients don't take their medications because they're unable to afford them, while others skip medication doses or delay filling a prescription to save money. This handout provides resources to assist patients with prescription access and cost savings. The following are Z codes that may be used in situations where medications are not taken due to financial hardship:
