BOULEVARD- Please have your forms filled out prior to your appointment. We are legally required to update your health history once a year, so if it has been a year since you filled it out-it's time! We have one i-pad for people checking in and out, it's not ideal for someone to be using it for 10-15 minutes filling out forms. Plus I have to do a new annual update, so I need it done prior so we can review any changes from last year. And just a pro tip from an old ICU nurse-keep a list of your medications on your person at all times!
