Special Care Organizational Record for Individuals with Special Health Care Needs
The Special Care Organizational Record (SCOR) is intended to help track and organize information in one central location. Using the SCOR will make it easier for someone else to step into the role of caregiver, if that is ever needed. Learn more about SCOR and how it can help you maintain records for your family member with special needs.

If you prefer a physical copy of an organizational tool for tracking medical or educational needs contact Families Together. One of our Parent Specialists will mail you a personalized copy of our CareING Notebook.
Preparing for Your Move
The "Preparing for Your Move" fact sheet for military families offers suggestions for parents of children with special needs to consider before, during, and after a move to a new duty station. Planning ahead helps ensure a smooth transition of special education and early intervention services. Learn more about this resource.
