June 2025 IDDSI e-bite

IDDSI is for all ages… a smaller world for babies and children!

Eating and drinking challenges (dysphagia) occur across all ages. IDDSI was developed to meet the needs of all ages, across all cultures and for all care settings.  Feeding disorders are a common challenge in babies and children.   Babies and children are much smaller than adults and do not have the same oral skills as adults; these skills develop over time as the child grows and develops.  Therefore, it was important to consider the unique needs of babies and children in the development of the IDDSI Framework.

So how is the IDDSI framework different for babies and children? While the name and description of the food and drink levels are the same, there are a few unique differences in terms of particle size recommended for babies and children. The smaller size of mouth, throat and airway and the gradual eruption of teeth and feeding skill were taken into consideration. A health professional with specialized knowledge and skills in assessing feeding disorders in babies and children should always be consulted to make the most appropriate recommendation of food texture and drink consistency.

L 5 Minced and Moist for babies and children have a particle size requirement of no larger than 2 mms and should still be soft and moist and pass the required testing methods for L5MM. L6 soft and bite sized for babies and children have a particle size requirement of no larger than 8 mm and should still pass the required testing methods for L6SB.

Many of you may not have been familiar with the Level 1, Slightly Thick drink consistency prior to the IDDSI framework being published. This consistency had been used more commonly with infants experiencing feeding challenges. The primary means of nutrition in the young infant is by sucking liquids, therefore it is essential the baby can do this efficiently and not tire. If the liquid is too thick and the baby fatigues, this can result in reduced intake and growth.

Keep in mind that although L1 slightly thick drinks and the smaller particle sizing for L5 Minced and Moist and L6 Soft and Bite sized were developed for babies and children, these can also be used in older populations if appropriate.  Handouts for babies and children can be found here on the IDDSI website.

Transitional Foods are also used commonly with babies and children, check out our July Ebite to learn more about this category.
