The collaboration between inpatient and outpatient teams, facilitated by complex care navigators and supported by effective communication, is vital for managing the complex needs of pediatric patients with chronic conditions.

 

Upon completion of this session attendees will be able to:

·        Identify purpose of outpatient complex care program

·        Identify role of inpatient complex care navigator

·        Identify utility of communication between inpatient and outpatient care management teams

 

Registration fee:

Announcement: Expansion of Case Management Certificate Program at Framingham State University


Framingham State University is pleased to announce the expansion of its Case Management Certificate Program to now include Social Workers, Physical Therapists, and Occupational Therapists. Originally launched in 2020 for Registered Nurses, this program was developed by Ellen Rearick, PhD, RN, CCM, who continues to lead and teach all courses.


The program is designed to be completed within one academic year (September to May), comprising two didactic courses in the Fall semester and one practicum course in the Spring semester. All classes are conducted remotely via Zoom, providing flexibility and convenience for working professionals.
