Team-Based Care:

A Guide for Family Caregivers as Partners

Family and friend caregivers are an important member of the healthcare team along with doctors, nurses, allied healthcare team members, and the person you are caring for. There are different ways you can participate, for example being present as support or participating in care decisions. How and when you participate will depend on the comfort levels of both you and the person you are caring for. Team-based care: a guide for family caregivers as partners flipbook is designed to help you know how to be a partner in care, learn strategies to feel comfortable in your role, and know what questions to ask and steps to take in your role as a partner in care.

Breath Work & Meditation zoom class

(Punjabi - Mar 21/ English - Mar 28) at 10:00am

In this monthly workshop on Zoom with the guidance of our licensed facilitator, get a profound understanding of breathwork (pranayama) and meditation and learn how to use your breath like a wonderful tool to start controlling all systems in your body.

Let's take care of our mind and be aware of inner consciousness to realize our true nature, incorporating personal intentions and spiritual practices acceptable to all. Focusing on intentional breathing patterns, altering the rate, ratio and flow of the respiratory cycle, we will use guiding techniques to enhance and minimize various sensory inputs through the use of breath, musical vibration and guided visualization.

Caregivers' Sharing

Caregivers Can Plan for the Unknown Future

We don’t have to like aging and what it may bring, but we can make caregiving easier with planning.

It will be impossible to plan for everything in the future, but family caregivers can still plan for what can lie ahead on a smaller scale. These important matters may be discussed verbally but writing plans down may be more beneficial. While a well-written care plan can take some time and effort to create, it can provide focus of what needs to be done as well as serve as a reminder of what was discussed, a means of organizing thoughts, and a road map of sorts to how things can be achieved.

Plans need to feel attainable. Starting small can be a good trick. List lesser caregiving tasks and share those responsibilities – one person doesn’t have to do everything. Perhaps one family member can pay the parental bills and another family member can balance the parental cheque book each month.
